IMMUNIZATIONS

 

New York State Recommended Childhood Vaccination Schedule

In order to start kindergarten your child must meet the minimum (your child may have more than the minimum) requirements for school entrance:

*Three doses of DPT.

*Three doses of OPV.

*Two doses of MMR.

*Three doses of Hepatitis B.

*Three doses of Tdap.

*One dose of Varicella (chickenpox) vaccine or doctor documented (date & signature) proof of disease. All children born on or after January 1, 2000.

*Since September 1, 2007, all students born on or after January 1, 1994 AND entering 6th grade on or after September 1, 2007 are required to have a booster shot of Tdap (Tetanus, Diptheria and Acellular Pertussis).

 

 

PHYSICAL EXAMINATIONS

New York State Education Law requires a physical exam of children when they:

*Enter a school district for the first time (including all kindergarten students).

*Are in grades 2, 4, 7 and 10.

*Participate in interscholastic sports (required yearly).

*Need working papers.

*Are referred by/to the Committee of Special Education.

*Are deemed necessary by school authorities to determine a child's education program.

  In April 2007 legislation passed requiring schools to include information on Body Mass Index (BMI) and weight status of students on individual health forms.   New York State Education law requires this information be completed by the student’s health care provider and submitted to the school by parents at school entry and in grades K, 2, 4, 7 and 10.

 

To be in compliance with the new regulation Cortland Enlarged City School District has updated our student health appraisal forms. Effective immediately health care providers will be asked to include the required information on the individual health forms that are maintained by the school district.   This does not require you to schedule any additional visits.   It will be collected at routinely scheduled appointments for your child.   For additional information please contact your school nurse or health care provider.


Attention Parents :

 

Cortland Enlarged City School District has been randomly selected by the state to provide estimates of childhood obesity rates at the county level.   The information will be taken from the health appraisals of students in grades Pre-K, K, 2, 4, 7, &10.   Each student’s health examination is required to include Body Mass Index (BMI) and determination of weight status.

If you do not wish your child’s data (no personal identifications) to be included in this survey please contact the nurse’s office by January 29th, 2010.
Addendum:  Also to be included are the following health conditions:  Asthma, Diabetes, High Cholesterol, and Hypertension.  Please notify the nurse by February 21st, 2010  if you do not wish your child's health condition from their physical to be part of the survey for the state. 

 

DENTAL HEALTH CERTIFICATE

 

New York State is requesting a Dental Health Certificate on all new entrants and students in grades K, 2, 4, 7 and 10.



MEDICATION

New York State Education Law requires that any student who needs to take medication during the school day provide:

*A written order from the physician for prescription and non-prescription medications.

*Medication in a properly labeled (pharmacy or original) container.

*A written request from the parent for administration of the medication.

SCREENINGS

New York State Education Law requires:

*Hearing screening to be performed annually to students in grades K,1, 3, 5, 7 and grade 10,

and to all new entrants.

*Vision screening to be performed annually to students in grades K, 1, 2, 3, 5, 7, 10, and all new entrants.

*Scoliosis screening must be performed annually to all students in grades 5 thru 9.

 

 

COMMUNICABLE DISEASES

Please do not send a sick or injured child to school with the intention that they will be diagnosed and treated by the nurse. The school nurse is not a medical diagnostician.

Any child displaying the symptoms of fever, vomiting, diarrhea, undiagnosed rashes, sore throat, ear pain, productive cough or wheezing, drainage from wounds, redness or drainage from eyes or active head lice should not come to school until seen, diagnosed and treated by their personal physician.
